  1. Use of the RBR: All healthcare providers, parents and caregivers are encouraged to use the RBR. The RBR has been made freely available since its original development in order to facilitate enhanced care of infants and children. The RBR has been endorsed by the Canadian Paediatric Society, the College of Family Physicians of Canada, and Dietitians of Canada. It can be downloaded free of charge from this website.

    Healthcare organizations and governments are encouraged to incorporate and support the use of the RBR to facilitate care of infants and children. Financial contributions to support ongoing RBR development projects are welcome. They can be made to the Memorial University of Newfoundland Rourke Baby Record Research and Development Fund, and directed to: Director of Administration, Office of Finance Faculty of Medicine, Medical Education Centre, 300 Prince Philip Drive, St. John’s NL A1B 3V6. All contributions will be acknowledged.

    Excerpts from this website may be copied or reproduced for print without permission for educational and non-commercial purposes. If quotes or excerpts are used, full acknowledgement must be given to the Authors. Full acknowledgement includes citing the full name of the Authors, the title and URL of the web page, and the date on which the information was captured.

  2. Provisos on modifications to the paper version of the Rourke Baby Record: Permission to modify the RBR is given consideration by the Authors, and permission has been granted with the following conditions:

    1. The record is clearly identified at the top of the sheet(s) as a modification from the 2020 RBR by the group involved including the date of the modification.
    2. As the College of Family Physicians of Canada (CFPC), the Canadian Paediatric Society (CPS), and Dietitians of Canada (DC) must approve any substantial change to the record before they endorse it, the CFPC, CPS, and DC logos must be removed from the modification unless endorsement of the modification has occurred. As well, ordering information at the bottom of the sheet may not be accurate for the modification.
    3. The contributor assigns all copyright to the RBR modification to the Authors and waives his/her moral rights to the modification.
    4. There is no fee charged to users for the use of the RBR modification.  The paper version of the RBR has always been free of charge in the public domain.
    5. There is no distribution of the RBR modification by commercial sources without written permission by Drs. Rourke and Dr. Leduc.
    6. Drs. Rourke and Dr. Leduc are sent a copy of the modified RBR in draft form for our review.
    7. Drs. Rourke and Dr. Leduc are sent a copy of the modified RBR in final form for our records.
    8. Permission is given to Drs. Rourke and Dr. Leduc to incorporate any improvements in the modification into future versions of the RBR.
    9. Permission is given to share the modification and to post it on the RBR website and other appropriate websites.

    Anyone seeking to contribute to or modify the RBR must request permission from the Authors, and in the request agree to abide by the conditions set forth above.  Permission can not be considered by the Authors unless the applicant has agreed to the above noted conditions.

  3. Provisos on modifications of the Rourke Baby Record for inclusion in EMRs: Drs. Rourke and Dr. Leduc recognize the increasing importance of EMRs in the facilitation of excellent clinical care. Many physicians and healthcare providers who have used the RBR in the past have adopted an EMR or will in the future use an EMR and wish to continue using the RBR in this format. Permission to modify the RBR for inclusion in EMRs is given consideration by the Authors with the following conditions:

    1. The record is clearly identified at the top of the sheet(s) as a modification from the 2020 RBR by the group involved including the date of the modification.
    2.
