Originally developed in 1979 by Drs. Leslie and James Rourke, family physicians currently living in St. John’s, Nfld., the latest revision was completed in 2011 in collaboration with Dr. Denis Leduc, a Montreal, Quebec paediatrician, who is a past president of the Canadian Paediatric Society.

is a family physician in St. John’s, Newfoundland, Canada and an Associate Professor of Family Medicine at Memorial University of Newfoundland. Prior to this, she practiced rural family medicine in Goderich, Ontario, Canada for 25 years with her husband/office partner, James Rourke. She obtained her MD (1977 cum laude), CCFP (1979), and Master of Clinical Science - Family Medicine (1996) all at The University of Western Ontario in London, Ontario, and became a Fellow of the CFPC in 1999. Throughout her medical career she has had a special interest in preventive health care of young children, and was an inaugural member of the Joint Action Committee on Child and Adolescent Health of the College of Family Physicians of Canada and the Canadian Paediatric Society. In 2002 she received The University of Western Ontario Martin J. Bass Recognition Award for exceptional contribution to family medicine for her development of both the Rourke Baby Record and also rural medicine and health curriculum. She was delighted to be awarded the 2006 Dr. Wallace J. Ingram Award for New Faculty at Memorial University to fund the development of this website.

is a community-based paediatrician in Montréal, Québec and an Associate Professor of Paediatrics at the McGill University Health Centre. After obtaining his medical degree from the University of Alberta (Edmonton), he came to McGill as a Robert Wood Johnson Clinical Scholar and obtained his Fellowship in Paediatrics and Certification in Family Medicine (1979). He is also a Fellow of the American Academy of Paediatrics (1982) and has been awarded Honorary Membership with the College of Family Physicians of Canada (2004). He is a Past-President and Life Member of the Canadian Paediatric Society and is the 2011 recipient of their Distinguished Community Paediatrician Award. His research interests include neonatal outcomes and their determinants and the standards of paediatric care in community settings.

has served as Dean of Medicine and Professor of Family Medicine at Memorial University of Newfoundland since his appointment in April 2004. He previously served as a Professor in the Department of Family Medicine and as the Assistant Dean of Rural Regional Medicine at The University of Western Ontario. He was an active rural family physician including obstetrics and emergency work in Goderich, Ontario for 25 years with his wife/office partner, Leslie Rourke. James Rourke is a recognized leader in rural medicine at provincial, national, and international levels, and has received many honours and awards, including the College of Physicians and Surgeons of Ontario Council Award (2004) "to honour outstanding Ontario physicians who have demonstrated excellence and come closest to meeting society’s vision of an ideal physician".

The Rourkes have received practical training in well baby and child issues over the years by their three children, Patrick, Ryan, and Shannon.
